Responsibilities

GENERAL PUPOSE:
Provides individualized assessment and treatment to restore, remediate or rehabilitate specific aspects of physical, cognitive, social, emotional and behavioral functioning and interests in leisure/play.
Conducts the assessment in a timely manner in accordance with standards and agency protocol.

JOB DUTIES

AND RESPONSIBILITIES:
Evaluates all patients referred, considering patient safety at all times.
Identifies the patient's strengths and limitations in physical, cognitive, social, emotional and behavioral functioning and interests in leisure/play.
Conducts the assessment in a timely manner in accordance with standards and agency protocol. 25%Plans and implements treatment for all patients using evidence-based practice and as determined by the evaluation at the recommended frequency that is appropriate and feasible, including functional interventions, therapeutic activities, recreation participation, leisure education, community skills training, activities with other interdisciplinary team members and peer support. 25%Plans and implements individualized and group treatment using appropriate treatment intervention and facilitation techniques. 20%Provides education to patients and families related to community (local and national) resources for use after discharge. 10%Documents evaluation findings/data, treatment program, Plan of Care/Patient Education, recommendation for frequency of therapy, patient progress, patient discharge summary, current physician orders and productivity statistics for all patients accurately and in a timely manner according to Departmental Policy and Quality Improvement measures. 10%Communicates with local community programs to assist patients seeking appropriate programs for continued recreational therapy and reintegrate into community life after discharge. 5%Consults with other disciplines and team members to promote the greatest degree of independent functioning possible within the patient's capabilities.
Participates in discharge planning by recommending equipment and community referrals, arrange teaching as appropriate or assigned.
Attends Health Team rounds as needed or assigned and attends family discharge planning conferences as necessary. 5%Other duties as assigned.
JOB SCOPE:
Moderate level of problem solving ability. Gathers and interprets data to solve routine problems that require verification.
Some independent judgement required.

QUALIFICATIONS

Minimum

Education

Required:
+ Bachelor's degree in Recreational Therapy or in Recreation with a concentration in Recreational Therapy.
Minimum

Education

Preferred:
+ Master's degree.
Requited

Experience
