The HSCS is comprised of nine psychiatric hospitals located throughout the state, and one youth residential psychiatric center in Waco. The HSCS is a hub of excellence for forensic mental health and complex psychiatric care, with all facilities accredited by The Joint Commission. The hospitals provide state-of-the-art treatment that is recovery-oriented and science-based. Whether in a direct-care or support role, your contribution will make a difference in the life of a Texan. The Nurse II works on Cottonwood, which serves as an Adult Inpatient unit. Varied shift openings. The Nurse II performs complex (journey-level) nursing work. Work involves providing for the assessment, care, and treatment of patients. Work may include serving as a resource on nursing practices and education programs by performing consultative and administrative work. May train others. Works under general supervision, with moderate latitude for the use of initiative and independent judgment. The Nurse II reports to the Nurse Manager. Serves as a mentor providing guidance and supervision to Nurse I, Licensed Vocational Nurses, and Psychiatric Nursing Assistants. Mentoring involves teaching new employees on the unit hospital procedures, guiding new employees on the unit through daily operations, and counseling all employees on the unit. Training employees daily unit operation procedures is sometimes a daily assignment due to the fact the employees from other units may be temporarily assigned to the unit to insure adequate coverage. Possesses a unique knowledge regarding the patient population served. Works under limited supervision with considerable latitude for the use of initiative and independent judgment. Assesses, plans, and directs the implementation of nursing care; performs nursing assessments; functions as a member of the interdisciplinary treatment team; provides direct care for individual patients; complies with established policies and procedures of the Nursing Services Department, the Hospital, and the agency. Promotes, establishes, and maintains the therapeutic milieu. Assists in teaching and directing other personnel. May be required to serve as house supervisor or coordinator. Must understand that duties, location of work and shift/pattern are subject to change as determined by the administration to meet the needs of the hospital. Must understand that duties may include exposure to blood or other potentially infectious materials. Will be available for on-call duty as designated. Under the direct supervision of the Nurse Manager, observes, assesses plans, intervenes and evaluates outcomes and documents all aspects of patient care. Possesses a unique knowledge regarding the patient population to be served. Coordinates scheduling within assigned areas. Provides primary proactive management for service area during assigned shift with a supportive but clear supervisory manner. Supervises, counsels, and evaluates other nursing staff. Collaborates with Nurse Coordinator, other charge nurses, and MH Supervisor/Specialist to ensure 24-hour continuity of care. Assumes duties of the Nurse Coordinator during absence. Is flexible in work schedule. Has broad latitude for initiative and independent judgment. Provides direct nursing care to individual patients as needed. Understands that duties, location of work and shift/pattern are subject to change as determined by the administration to meet the needs of the hospital. Essential Job Functions: Attends work on a regular basis and may be required to work a specific shift schedule or, at times, even a rotating schedule, extended shift and/or overtime in accordance with agency leave policy and performs other duties as assigned.
1. Assess the health/mental status of the patient upon admission and on an ongoing basis and documents assessment, planning, intervention and evaluation in patient record in order to determine priorities and plan therapeutic nursing care. Performs CPR/Heimlich techniques correctly and safely according to the specifications of the American Heart Association. Review medical records and patient assessments to determine compliance with policies and procedures, correctness of assessments, and quality of services.
2. Administers medications as ordered by various methods using the 6 Rights and transcribes and notes orders related to medication administration in order to provide safe medications administration.
3. Completes medication administration record and completes control drug count and record following established policies and procedures in order to comply with federal standards and regulations and ensure accurate and complete documentation of the medication administration process.
4. Implements special precautions as ordered and explains and documents rights restrictions and implements restraint/seclusion procedures according to departmental policy and established nursing standards in order to maintain patient safety, protect the patient from causing harm to self and/or others and uphold patient rights.
5. Provides and documents individual/group teaching to patients regarding medications, illness, special treatment, etc. utilizing appropriate teaching/response methods for various age groups and cognitive levels in order to facilitate patient learning. Provide consultation, training and technical assistance to agency staff, care recipients, provider agencies, the medical community and other organizations. Provide specialized nursing services for individuals, families, group and communities.
6. Attends mandatory training in order to ensure staff competence and development. Attends and participates in on-going in-service and other education and training programs appropriate to the population/age group in order to meet the physical, psychosocial, and cultural needs of patients. Makes recommendations to Nursing Service for improvements.
7. Utilizes therapeutic communication skills in interactions with patients in various age groups in a caring, respectful manner in order to promote the healing process and return of the patient to the least restrictive environment.
8. Carries out physicians orders and implements nursing interventions according to acceptable nursing care standards and hospital policies and procedures. Plans, develops, implements interprets and monitors program operations, policies and procedures.
9. Monitors TIPS techniques 1 through 6 and, in the absence of other staff, implements TIPS techniques 1 through
3. 2 in order to ensure the well-being of patients and staff.10. If working nights, maintains a quiet and restful environment to promote patients rest and sleep by utilizing therapeutic interventions with patients exhibiting altered sleep patterns.11. Will observe, promote and obey the safety rules applicable to all aspects of facility and department operations as described, (or referenced, in the Safety Management Program SOP 300-41) to ensure no injuries occur to self, other staff, consumers and visitors due to the failure to follow facility standards.12. Acts as a mentor, providing guidance and supervision to less experience or less-educationally prepared Nurse I. Reviews and contributes to evaluating the performance of Nurse I, LVN, and Mental Health Workers. Serves as on-call nurse as scheduled.
Other duties as assigned include but are not limited to actively participating and/or serving in a supporting role to meet the agencys obligations for disaster response and/or recovery or Continuity of Operations (COOP) activation. RequirementsLicensed as a Registered Nurse (RN) or be eligible for licensure in the State of Texas. Plus, three (3) years of work experience as an RN in a recognized health agency, hospital, or health care facility. Or a bachelor's degree in nursing from an accredited program, plus two (2) years of work experience as an RN in a recognized health agency, hospital, or health care facility; or a master's degree in nursing from an accredited program plus one (1) year of work experience as an RN in a recognized health agency, hospital or health care facility. Initial Selection Criteria: Licensed as a Registered Nurse (RN) or be eligible for licensure in the State of Texas. Plus, three (3) years of work experience as an RN in a recognized health agency, hospital, or health care facility. Or a bachelor's degree in nursing from an accredited program, plus two (2) years of work experience as an RN in a recognized health agency, hospital, or health care facility; or a master's degree in nursing from an accredited program plus one (1) year of work experience as an RN in a recognized health agency, hospital or health care facility.
